FiveM whitelist application template
A whitelist application is the first thing a new player sees from your server, and the first filter your staff team has. This is the template FiveApply ships with: the questions, why each one is there, and how to review the answers. Copy it into any form tool, or use it in FiveApply with one click.
What a good whitelist application does
- Checks effort, not writing talent. A short, honest backstory beats a polished one copied from a forum.
- Tests rule knowledge. A few quiz questions on RDM, metagaming, NLR and value of life catch players who skipped the rules.
- Shows how someone roleplays under pressure. One scenario question tells you more than three paragraphs of backstory.
- Stays short enough to finish. Aim for 10–15 minutes. Long forms put off good players and don't stop bad ones.
The template: Civilian Whitelist
The main whitelist for new players: character backstory, RP experience and a short rules quiz. It's split into out-of-character (OOC) questions about the player and in-character (IC) questions about their character.

Recommended review settings
The template comes with these defaults. Change any of them to suit your server.
| Votes needed | 2 staff votes, majority decides |
|---|---|
| Quiz pass mark | 75%. Below that the application is flagged for staff, not auto-denied |
| Reapply cooldown | 7 days after a denial |
| Discord account age | At least 30 days, to keep out throwaway alts |
| On approval | Give the whitelisted Discord role, which txAdmin or your connect script checks |
Tips for reviewing whitelist applications
- Watch for copied backstories. Players reuse stories from other servers and from each other. FiveApply compares every long answer against earlier applications and flags close matches.
- Read the scenario answer first. “I'd hand over the keys, my character isn't stupid” is value of life. “I'd pull my gun” is a red flag.
- Use more than one reviewer. Two or more votes, with votes hidden until you've cast yours, stops one person waving friends through.
- Deny with a reason. A short, specific reason (“backstory too short, add your character's goals”) brings back better second attempts.
- Only store what you need. Many applicants are under 18. Ask for an age, not a date of birth, and delete denied applications after a while.
